By accessing this website, scheduling an appointment, or receiving services from TrueNorth Physical Therapy, PLLC, you agree to these Terms & Conditions.
Services Provided
TrueNorth Physical Therapy, PLLC provides physical therapy, wellness, and related services, including in-person and telehealth visits. Services are delivered by licensed physical therapists and are intended to support movement, function, and overall physical health.

Services do not replace medical care from a physician, nor do we provide emergency medical services.
Cash-Based Practice
Our clinic operates on a cash-based, self-pay model:

Payment is due at the time of service

We do not bill insurance on your behalf

We may provide receipts or superbills upon request

It is the patient’s responsibility to verify any out-of-network benefits
Appointments, Cancellations, and No-shows

Appointments must be cancelled or rescheduled at least 24 hours in advance

Late cancellations or no-shows may result in a fee of $150

Fees are the patient’s responsibility and will not be billed to insurance

Repeated no-shows may result in discharge from care
Payment Terms

We accept credit/debit cards, HSA/FSA cards, cash or checks

By providing payment information, you authorize TrueNorth Physical Therapy, PLLC to charge applicable fees, including cancellation or no-show fees, according to clinic policy
Refund Policy

Services rendered are non-refundable

Prepaid services or packages may be subject to specific refund terms outlined at time of purchase

Missed appointments are not eligible for refunds
Telehealth Policy

Telehealth services are provided when appropriate and may involve inherent risks related to technology

By participating in telehealth, you acknowledge and accept these risks.

Telehealth may not be suitable for all conditions, and in-person visits may be recommended
Medical Disclaimer
Physical therapy involves physical activity and manual techniques that carry inherent risks. By receiving services, you acknowledge:
